1、你哪儿不舒服?Nǐ nǎr bù shūfu?
Where does it not feel good?
The doctors usually would probably ask you你哪儿不舒服?
你 nǐ, you.
哪儿 nǎr, where.
不 bù, not.
舒服 shūfu, the first tone and the second tone, it means comfortable.

2、我头疼。Wǒ tóu téng.
I have a headache.
我头疼, which means I’ve got a headache.
头 tóu, means head.
疼 téng, ache.

3、我拉肚子。Wǒ lā dùzi.
What if you have diarrhea?
How should you tell the doctor about that? You can say 我拉肚子.
拉肚子 lādùzi, have diarrhea.

4、先量一下体温。Xiān liáng yíxià tǐwēn.
First to take a temperature.
If you have a cold, the doctor might ask you to take a temperature. 先量一下体温.
体温 tǐwēn, body temperature.
量体温 liáng tǐwēn, to take a temperature.
一下 yí xià, is normally used as an adver after a verb indicating the action.
量一下体温 liáng yí xià tǐwēn, to take a temperature.
先 xiān, first.

5、什么时候开始的?Shénme shíhou kāishǐ de?
When did it start?
Then the doctor might ask you “When did it start?” In Chinese it’s 什么时候开始的?
什么时候 shénme shíhou, when.
开始 kāishǐ, start.
